Initial Comments:
Pursuant to information received by this field office dated December 21, 2023, pertinent information has been forwarded to the Office of Long-Term Living, State Medicaid Agency regarding a decrease of one (1) bed at this facility. This will reduce the overall capacity at this facility from the current five (5) ICF/IID certified beds to the proposed four (4) ICF/IID certified beds. This decrease is effective January 1, 2024 as requested in facility correspondence.
